Information

Welcome to St Anns

If you have a health problem we would like to help.

In this Practice you have a Doctor and Nurses working in Primary Care.

You can think of health care in the UK like a ladder, steps:

5. Secondary care – Hospitals (A & E only to be used for accidents and emergencies)
4. Primary care – e.g: Family Doctors (GP’s) & Nurses, Health Visitors (for children) and midwives.
3. Advice from Professionals – Pharmacy first & NHS Direct: 0845 4647
or www.nhsdirect.co.uk or walk in Center London Road Notthingham.
2. Advice – From Friends and Family.
1. Self Care – Look after yourself.

Appointments

Your appointment may be urgent and need attention the same or next day, or it may be important enough to wait a few days until a 10 or 15 minute appointment can be made. It is important you keep your appointment. If for any reason you need to cancel please call the surgery in advance.

If you need an urgent appointment please phone first thing in the morning before 10:00AM.

Your Appointment Is Important

If you have an interpreter booked and need to cancel your appointment. Please give 24 hours notice, as we still have to pay the interpreter.

Doctors mainly make a diagnosis from what you tell them and then do an examination, also sometimes tests to confirm what they (or you) suspect.

Before then, please think about these questions (you can wrote on the sheet if it helps you remember).
Q1: What is the most important reason for your visit?
Q2: What other things do you want to ask about?
Q3: How long have you had these problems?
Q4: What medicines are you taking (including Traditional and Herbal cures)?
Q5: What do you think might be causing these problems?
Q6: Do you have any stress that might affect you physically?

Please ensure that you can only make an appointment between 4:PM – 5:PM
